Thank you for your interest in the Be Connected Assistance Program. This page includes a link to our updated form for service members, veterans & their families to be referred for assistance. There is a 2-step process for requesting assistance:
Step 1: Assistance Referral – To make an assistance referral for yourself or someone you are helping, please start by completing the referral form linked below or you can call 866-4AZ-VETS (429-8387). The new form requires less detail initially than the previous form.
Step 2: Assistance Request – Once the referral is received, our Be Connected team will follow up by phone and gather additional information to submit the full request if the individual or family is eligible. This program is confidential and provided at no cost to participants. Proof of military service, identification and documentation will be required. Please note: All requests require 5-7 business days to process once the full request has been submitted.
The BC Assistance Program is a fund of last resort and it can provide connection to resources and help for an array of needs.

- Resources for rent/mortgage assistance
