Make a Referral to Be Connected

Take the first step toward connecting yourself or someone you love resources and support. Make a referral for yourself or someone you care about to the Be Connected program that offers confidential, no-cost support to service members, veterans, and their families. If you or someone you care about needs help, reach out. Our straightforward referral process connects you or someone you care about to essential resources efficiently and privately. For more urgent needs, our support line is available Monday – Friday from 8 AM to 5 PM at 866-4AZ-VETS (866-429-8387).

Frequently Asked Questions

Be Connected services are available at no cost to Arizona service members, veterans and their family members. We also have services for helpers, organizations, and communities.

Anyone is welcome to complete a Be Connected referral form for themselves or on behalf of someone else who may need assistance and resources.

  1. To start the process, complete a referral form.
  2. Select “Support Line” for the type of referral. Then select “Standard Support Line referral.”
  3. Complete the form with as many details as possible. 
  4. Once the form is submitted, a member of the Be Connected team will reach out within two (2) business days.
  5. The team will make up to three (3) attempts to contact the person. 

Coaching Into Connection

60% of Americans feel lonely. The Coaching Into Connection program is here to help strengthen the social network for service members, veterans and their families at no cost. Care Transition

Support after a major life transition can be crucial, and the Be Connected Care Transition program is here to help Arizona service members, veterans, and their families navigate these challenging times. To get started, complete a Care Transition referral.
