Be Connected: Now Accepting Online Referrals!

Be Connected services are available to all Arizona service members, veterans, and family members. Our team is standing by to provide resource connection and navigation.

General Referrals:

Anyone is welcome to complete a Be Connected referral form for themselves or on behalf of someone else who may need a resource connection and navigation assistance. To start the process, please complete a Be Connected referral form. For Type of Referral, select Support Line, and for Support Line Service, select Standard Support Line referral. Complete the remaining fields on the universal referral form with as many details as possible. 

Next Steps: A member of the Be Connected team will reach out within two business days. They will make up to three attempts to contact the person.

Coaching Into Connection – 60% of Americans feel lonely and our Coaching Into Connection program is here to support service members, veterans & their families in strengthening your support/social network. The team provides services statewide at no cost. To start the process, please complete a referral form.

Care Transition Services – The Care Transition team is here to support service members, veterans & their families who are in situations requiring additional care navigation support, including post-emergency room and hospitalization, situations where a person or family is unhoused or in housing transition, justice-involved and either existing or awaiting treatment for mental health or substance use. To start the process, please complete a referral form
